PROJECT PROPOSAL: Establishment of a Biomedical Engineering Orthopedic Rehabilitation Centre in Southwestern Uganda

1. Executive Summary

This proposal presents a strategic initiative to establish a Biomedical Engineering Orthopedic Rehabilitation Centre in Mbarara, a growing urban and medical hub in Southwestern Uganda. The project aims to bridge the critical gap in orthopedic care, prosthetic and orthotic services, and biomedical rehabilitation using innovative and affordable technologies. By integrating modern biomedical engineering solutions with clinical rehabilitation, the center will provide specialized services to patients with musculoskeletal injuries, amputations, neurological conditions, congenital deformities, and age-related disabilities. It will also serve as a hub for biomedical research in orthopedics, local innovation, and capacity building in rehabilitation technologies.

2. Problem Statement

Uganda faces a rising burden of orthopedic conditions and physical disabilities caused by road traffic accidents, occupational hazards, congenital deformities, chronic diseases, and aging. In Southwestern Uganda, particularly in rural and peri-urban areas, access to quality orthopedic and rehabilitation services remains extremely limited. • Most health facilities lack specialized rehabilitation equipment and trained personnel. • Patients must travel long distances to Kampala or even outside the country for prosthetic, orthotic, and rehabilitation services. • There is limited integration of biomedical engineering technologies into Uganda’s health system. • There is a growing number of young people with amputations and spinal injuries due to road traffic accidents, many of whom are left economically unproductive due to lack of rehabilitation.

3. Project Goal

To establish a state-of-the-art Biomedical Engineering Orthopedic Rehabilitation Centre in South Western Uganda to provide accessible, affordable, and innovative orthopedic and rehabilitation services to underserved populations in Southwestern Uganda.

4. Objectives

1. Clinical Services: To provide high-quality orthopedic care, prosthetics, orthotics, and rehabilitation therapies to patients of all ages.

2. Innovation and Research: To support biomedical engineering innovations that solve local rehabilitation challenges.

3. Training and Capacity Building: To train biomedical engineers, rehabilitation therapists, and technicians in modern rehabilitation technologies.

4. Community Outreach: To provide mobile outreach services to rural and underserved areas.

5. Partnerships: To collaborate with hospitals, universities, NGOs, and government institutions to strengthen rehabilitation services in Uganda.

5. Target Population

• Individuals with physical disabilities (amputees, spinal cord injury, cerebral palsy, stroke, etc.)

• Victims of road traffic accidents and trauma • Children with congenital deformities • The elderly with mobility impairments

• Veterans and persons with disabilities (PWDs) • Rural and poor patients unable to access services

6. Project Components

 A. Infrastructure Development • Construction or leasing of a facility in South Western Uganda Preferably Mbarara town or near Mbarara Regional Referral Hospital

• Design of specialized departments: Orthopedic Clinic, Prosthetics & Orthotics Workshop, Physiotherapy & Occupational Therapy Rooms, Assistive Technology Lab

B. Equipment Procurement

• 3D Scanners and printers and CNC machines for custom prosthetic and orthotic manufacturing • Rehabilitation and physiotherapy equipment (treadmills, therapy beds, robotic exoskeletons, etc.)

• Diagnostic tools (X-ray, gait analysis, pressure mapping systems)

• Patient management software and data systems

C. Human Resource Development • Recruitment and training of biomedical engineers, orthopedic surgeons, prosthetists/orthotists, physiotherapists, and community health workers • Internships and residencies for biomedical engineering students from local universities

D. Service Delivery • Fabrication and fitting of prosthetic and orthotic devices • Rehabilitation therapies (physical therapy, occupational therapy) • Follow-up care and home-based rehabilitation • Mobile clinics for outreach in rural areas

E. Research and Innovation Hub • Collaborative research with universities (e.g., MUST – Mbarara University of Science and Technology)

• Development of affordable assistive technologies using local materials • Innovation challenges and incubation of rehabilitation start-ups

7. Justification

• High Demand: Road traffic injuries are the leading cause of disability in Uganda. Rehabilitation services are in urgent demand.

• Strategic Location: Mbarara is a growing regional medical hub with a large catchment area including all districts around it.

• Innovation Potential: Uganda’s young biomedical engineering workforce can create local solutions with global impact.

• Sustainability: Through service fees, insurance, and partnerships, the center can become financially sustainable within 3–5 years.

8. Partnerships and Stakeholders

• Local: Mbarara Regional Referral Hospital, MUST, Uganda Allied Health Professionals Council, Ministry of Health

• International: Spiritual Family Network and seeking for more international partnerships

• Private Sector: Looking forward to collaborate with Medical device companies, NGOs, insurance providers

10. Sustainability Plan

• Fee-for-service model for middle-income clients

• Insurance partnerships (e.g., UHI, Jubilee, community health schemes)

• Donor and CSR support for indigent patients • Revenue from custom assistive device sales

• Training and consultancy services

• Research grants and innovation competitions

11. Monitoring and Evaluation

• Baseline data collection at inception •

Quarterly reports on patient outcomes, services delivered, and financial performance

• Independent annual evaluations • Continuous stakeholder engagement and feedback mechanisms

13. Conclusion

The establishment of a Biomedical Engineering Orthopedic Rehabilitation Centre in Western Uganda is a transformative project that addresses a critical and growing need. With strategic investment and support, this center will improve lives, restore mobility, foster innovation, and create a model for disability-inclusive healthcare in Uganda and beyond.
